It covers national and international policies, various disability and healthcare models, shared decision-making, patient empowerment, health behavior change models, and evidence-based practices for improving and evaluating long-term condition services.

  • In addition, applicants must demonstrate:
  • Relevant work experience,
  • Relevant professional qualifications (if applicable),
  • Satisfactory performance in their previous studies, and
  • Strong personal statement highlighting their interest in the module and its relevance to their career aspirations.

International Overseas Students (Outside EU):

  • Applicants must possess a minimum of a Lower Second Class Honours degree in a subject relevant to the intended long-term condition.
  • Additionally, applicants must:
  • Meet the English language proficiency requirements (see Language Proficiency Requirements below).
  • This can be demonstrated through achieving the following minimum scores in recognized English language tests:
  • IELTS: 6.5 overall (with no sub-score below 6.0)
